2009-10 Calendar and Handbook

The 2009-10 Calendar and Handbook contains the calendar for the school year as well as information about our school. Included are: Admission and attendance policies, academic policies, the school dress information, discipline policies, financial information and other school policies.

2009-10 Registration Forms

All Students:

• Student Application Form (one per student)
• Financial Agreement Form (one per family)
• Consent for Treatment Form (one per student)
• Computer Acceptable Use Form (one per student)
• Music Registration Form (one per student)
• After School Care Registration Form (if applicable)
• Emergency Card (one per student) - must be picked up from the school office

New Students:

• Student Recommendation Form (three per student from former school: principal, teachers, ect.)
• Physical Examination Form (all kindergartners and new students coming from out of state)
• Request for Records (one per student)
